Pan-American Square Apartments – Substance Use Disorder Single-Site Supportive Housing
Pan-American Square Apartments Substance Use Disorder (SUD) Single-Site Supportive Housing Program is a New York State Department of Health funded program that provides long-term or permanent housing where residents can access the support services they require to live successfully in the community.
Located on Delaware Avenue in Buffalo, New York, the Pan-American Square Apartments include 150 apartments, five of which are reserved for individuals with a substance use disorder to receive services from the Supportive Housing Program.

Services provided:
Tenants will be assigned to a housing specialist on-site to assist with identifying service gaps and subsequent linkages in the community, providing advocacy, networking with service providers, and support in maintaining independence through creation of a housing support plan.
Admission Criteria:
- Have a primary diagnosis of a substance use disorder (SUD).
- Be in a housing crisis without a permanent residence.
- Possess independent living skills to maintain an apartment. There is no medication room on site.
- Have a desire to participate in rehabilitative goal planning to increase independence.
- Must meet eligibility requirements of the Low-Income Housing Tax Credit Program. Income and occupancy requirements apply.
Program includes:
- Rental assistance – Tenant pays 30% of their adjusted income or Department of Human Services (DHS) shelter rate, an agency rental stipend pays the remainder of the rent.
- Security deposit is not required.
- A fully furnished apartment, including necessities such as linens, dishes, silverware, pots, pans, vacuum and cleaning supplies.
- Heat, air conditioning, hot water and electric are included in the rent.
The buildings are 100 percent smoke free. There are designated smoking areas provided outdoors.
